EVMS Occupational Health needs a written request to release documentation of medical information from an EVMS Occupational Health record.
Complete the Authorization for the Release of Medical Information form and forward to Occupational Health.

***EVMS Occupational Health can only accept requests for EVMS Occupational Health records. Requests for patient medical records must be sent directly to the Medical Practice of the patient.
