Ask your doctor for a referral if you:

• Are a diabetic patient that requires yearly examinations
• Have a family history of eye disease (glaucoma, macular degeneration/etc.)
• Are an older patient with visual complaints to evaluate for cataracts:
  • Difficulty with night vision/driving
  • Difficulty reading
• All sickle cell patients, especially sickle cell disease
• Have signs of eye infection refractive to treatment or atypical in presentation
• Are diagnosed with juvenile rheumatoid arthritis (JRA),rheumatoid arthritis (RA), sarcoidosis, inflammatory bowel disease or other rheumatologic diseases to screen for uveitis
• Those who use ethambutol or plaquenil and require baseline and ongoing screening examinations
• Have visual complaints such as flashes, dimming of vision or missing part of the visual field require urgent evaluation to rule out retinal detachment
