Efficacy of cosmetic products in cellulite reduction: systematic review and meta-analysis

 In recent years there has been a dramatic increase in the number of cosmetic products that are marketed as treatments for cellulite.  This study published out of Milan Italy in 2013 sought determine the efficacy of individual cellulite treatment products.  This study was a review article looking at all the currently published that evaluated topical products to treat cellulite. The authors found 21 studies that met the criteria that they decided should could be included in the systematic review.  Of these 21 studies, 7 controlled trials using thigh circumference as the main outcome measured were included in the meta-analysis.  The methods of the various studies were too variable among the published articles that were reviewed to draw any definite conclusions in comparing products, but xathenes, retinoids, and botanicals seem to have moderate efficacy in treating cellulite.  The act of tissue massage in applying the cream could account for some of the beneficial effects.  This study shows the need for future randomized controlled trials in order to better evaluate and compare these products. Treatment of Cellulite with LPG endermologie

LPG is an electrically powered hand-held machine that has two rollers and a vacuum chamber. The device provides positive pressure to skin and subcutaneous tissue by rhythmic folding and unfolding between rollers as well as negative pressure via aspiration. Originally made to treat trauma and burn scars. During use physicians noticed that it reduced the appearance of cellulite. Theorized to stretch the vertical connective tissues and stimulating lymphatic flow. Overall LPG is well tolerated and effective in reducing diameter of body contours.  Treatment for Cellulite: Varies treatment modalities 

Topical agents: Show little to no long-term benefit. Energy-based devices: Lasers, light, radiofrequency (RF), acoustic waves. The main effective laser treatment has been the side-firing fiber 1440-nm Nd:YAG laser. One-time treatment very well tolerated. Improvement of dimples and contour irregularity. Lasting up to 6 months. Radiofrequency devices deliver thermal energy to dermal/subcutaneous plan via electrodes. Causing rise in tissue temperature, collagen denaturation, remodeling neocollagenesis, and lipolysis. The Velasmooth and Velashape systems combine infrared light, bipolar RF, and mechanical manipulation of the skin with suction and massage with improvement in thigh circumference and cellulite. In the US the Venus legacy devices improved abdominal cellulite. 

Acoustic wave therapy causes pressure waves to subcutaneous tissue to promote lipolysis, improve local blood flow, enable lymphatic drainage and production of new collagen. Two types of acoustic waves focused shock (ESWT) and radial shock waves. Mild improvement of body contour and appearance of cellulite. 

Subcision: Area treated is numbed with topical anesthetic, a needle is inserted under the skin, and a fanning technique is used to release fibrous cords of cellulite. Cellafina has been shown to improve cellulite with results lasting > 3 years. Overall a combination approach would yield a higher patient satisfaction rate in the appearance of cellulite.
