Students will learn the practices of Hotspotters - how to identify patients in communities with multiple complex health conditions who are high-utilizers of care. Students will design care systems that will identify the root cause of patients' healthcare issues, identify how socio-demographic factors may play a role and develop patient options for becoming low-utilizers of care.
Students will conduct an initial assessment, identify priorities and plan action steps.
Students will:
- Identify patients who would benefit from Hotspotters.
- Establish relationships with patients who would benefit from Hotspotters.
- Describe barriers to care with these patients.
- Create a care plan with patient, setting achievable goals.
- Create links to social supports and primary physician in care plan.
- Describe and demonstrate value for data’s role in healthcare intervention.
- Defend the importance of coordinated care and patient-centered approach.
- Describe the economic benefits of coordinated care and patient-centered approach.
- Demonstrate a value for interdisciplinary collaboration.
- Report on patient experiences and stories.
- Develop a health map or diagram of the system influences on health for at least one patient participant.
