EVMS Family & Community Medicine earn re-certification as a PCMH

Congratulations to EVMS Ghent Family Medicine and EVMS Portsmouth Family Medicine for earning re-certification as a Patient-Centered Medical Home (PCMH).
The National Committee for Quality Assurance (NCQA) defines the Patient-Centered Medical Home as “a model of care that puts patients at the forefront of care. PCMHs build better relationships between people and their clinical care teams. Research shows that they improve quality, the patient experience and staff satisfaction, while reducing health care costs”
By earning re-certification recognition, both EVMS Ghent and EVMS Portsmouth Family Medicine have made a commitment to quality improvement using a patient-centered approach to care that result in patients that are happier and healthier.
