Geriatric services are provided across a spectrum of care in a variety of settings, which include adult-care residences; nursing, rehabilitation and long-term care centers; the medical office; and the hospital.
Geriatric Assessment Consultation: Geriatric assessment focuses on the physical, cognitive, psychological and social aspects of an older person’s life, in order to optimize self-care ability and quality of life. The first step is an accurate diagnosis, especially in patients with medical complexity. The consultation provides guidelines for treatment, living situation, and caregiving issues.
Palliative Medicine Consultation: For patients approaching end-of-life or with distressing symptoms, the Glennan Center directs and staffs the Palliative Care Consultative Service with Sentara Healthcare at Sentara Norfolk General Hospital. Available at Sentara Life Center, Life Care Facility at Newtown, Beth Sholom and Harbor’s Edge.
Memory Consultation Clinic: Often a concern of aging patients, forgetfulness and mild cognitive impairment are being successfully treated at the Glennan Center’s flagship outpatient program, the Memory Consultation Clinic. Available Monday, Wednesday and Friday, clinic personnel evaluate the extent of memory loss, look for the cause and suggest ways patients and their families can plan for the future.
ACE Unit: A Glennan innovation is the ACE Unit, a nationally recognized model created by Dr. Robert Palmer and Dr. Seth Landefeld, which offers enhanced care for older adults in specially designed hospital units, delivered by an interdisciplinary team of medical professionals, including geriatricians, advanced practice nurses, social workers, pharmacists and physical and occupational therapists. The care in an ACE unit is cost effective, compassionate and patient-centered, with a demonstrated measurable reduction in the length of hospital stays for these patients. This unit is typical of the team approach and attention to detail that mark elder care from EVMS. Available at Sentara Norfolk General Hospital and Sentara Careplex.
